Strangulation doesn't seem to be doing anything really

Hit someone with blood claws and the Strangling buff will show up but after that I do not see any death damage, just the normal unholy, and no extra stacks of the buff appear in any way I can see for subsequent hits. All up nothing seems to happen but the message "You focus your malice on choking their life away appearing" appearing on each attack while I have the buff.
